The Challenge
CarpenDiem is a concierge wellness clinic serving Orange County — think IV therapy, hormone optimization, aesthetic treatments, weight-loss protocols and the kind of personalized longevity care that normally lives behind a Beverly Hills storefront. Their patient population is bilingual by default: a mix of Spanish-speaking first-generation Orange County residents and English-dominant professionals from Irvine, Newport Beach and the broader South OC corridor.
The site they came in with did not reflect any of that. It was a generic WordPress template — the kind that gets re-skinned for dentists, salons and medi-spas without any particular care for what the business actually does. The specific problems TopWebLA documented at the start of the project:
No real bilingual experience. English only, with a Google Translate widget bolted on. Spanish-speaking patients who found the clinic through a referral had no way to read service descriptions, consent language or pricing in their own language.
Thin medical structured data. No MedicalBusiness schema, no MedicalProcedure schema on treatment pages, no explicit specialties list. Google and the AI search engines had no clean way to understand that this was a medical practice as opposed to a day spa.
Weak Google Business Profile tie-in. GBP existed but was disconnected from the site — no NAP consistency, no structured hours, no service list syncing back to the clinic's directory presence.
Mobile performance in the 50s. Heavy page builder bloat, uncompressed hero images, blocking third-party scripts. On a mid-range phone over 4G, the hero took more than five seconds to paint.
Zero AI-engine readiness. No llms.txt, no clear "about" summary for citation, no entity disambiguation. ChatGPT, Claude and Perplexity had nothing to work with when a user asked about "medical wellness clinics in Orange County."
The brief from the founders was quiet and specific: build something that looks like the clinic actually is — serious, concierge-grade, bilingual by default — and make sure both Google and the AI engines understand what we do.
The Approach
TopWebLA treated this as a rebuild rather than a refresh. The decision was made in the first discovery call to abandon WordPress entirely and ship a hand-coded static site, hosted on a modern CDN, with every piece of content authored twice — once in English, once in native Spanish, with a real toggle rather than a translate script.
Information architecture
We mapped the clinic's offering into four core service families: aesthetic treatments ("look your best"), medical wellness ("feel your best"), weight-loss protocols, and the full-service transformation program. Each family got its own hub page with native-Spanish counterparts, so that a Google query like "pérdida de peso Orange County" could land directly on a Spanish-first page — not a translated afterthought.
Technical SEO + GEO stack
Every page ships with MedicalBusiness and MedicalClinic schema, with explicit medicalSpecialty tags, acceptedPaymentMethod, and areaServed set to Orange County. Service pages carry MedicalProcedure and MedicalTherapy objects where appropriate. The site publishes an llms.txt file that spells out — in plain English — what the clinic does, who it serves, and how to reach it, so AI search engines have a canonical description to cite.
Performance
The team rebuilt the hero section from scratch with modern <picture> sources, preloaded only the above-the-fold critical assets, inlined critical CSS, and deferred everything else. Third-party trackers were consolidated. The result is a site that lands in the 90s on mobile Lighthouse — a measurable, Google-rewarded gap over the previous build.
The Delivery
CarpenDiem took the custom bilingual scope — not an off-the-shelf package. The project ran roughly three weeks end to end, with the following included in scope:
Hand-coded static site (HTML/CSS/JS, no CMS)
Approximately 15 production pages across the two languages (English + Spanish mirrors)
Hero, services hub, individual service pages (feel-your-best, look-your-best, lose-weight, your-transformation, weight-loss-plans), staff page, grand opening page, thank-you flow
Real EN/ES toggle with Spanish-first authored content
MedicalBusiness, MedicalClinic, Physician and MedicalProcedure schema across the site
llms.txt file for AI search engine citation
Google Business Profile audit + optimization pass (hours, services, photos, NAP consistency)
Booking integration surfaced on every service page
Mobile-first design pass with PageSpeed-focused image compression and asset preloading
TopWebLA shipped carpendiem.com in 2026 as a hand-coded bilingual medical wellness site. Measured against the previous WordPress build, the technical gains are specific and verifiable from the live site:
Mobile PageSpeed 90+. The current build sits in the 90s on Lighthouse mobile — up from the 50s on the previous WordPress template. Desktop scores sit above 95.
~15 pages indexed across EN + ES. Each service and each hub exists in both languages, roughly doubling the surface area Google can rank against bilingual queries.
MedicalBusiness schema on every page. The clinic now shows up with rich-result eligibility for medical queries — hours, services, specialties, accepted insurance, all structured.
llms.txt live. ChatGPT, Claude and Perplexity now have a canonical, machine-readable description to cite when users ask about "medical wellness in Orange County" or "concierge healthcare Irvine."
GBP consistency. The clinic's Google Business Profile, website and schema now share one NAP, one set of hours and one service taxonomy — the baseline Google rewards for local pack placement.
Specific lead-flow and booking metrics belong to CarpenDiem and are not published here. What TopWebLA documents publicly is the technical delivery and the live state of the site — both of which can be verified at carpendiem.com.
"We wanted a site that looked like what the clinic actually is — not another cookie-cutter medi-spa template. TopWebLA understood that from the first call, and shipped something our bilingual patients can actually read."
